New Company Name – New Look and Feel

The sale of Talcott Resolution was successfully completed on May 31, 2018. Talcott Resolution is now an independent, stand-alone insurance company. As a result, written and emailed communications, as well as our website, have been updated to reflect the Talcott Resolution name and new logo.

Updated Annuity Forms

Coinciding with these updates, our administrative forms have also been consolidated and simplified. Certain commonly used Talcott Resolution annuity forms are located on our new website, www.talcottresolution.com. From the website homepage, click on “Download Forms” under the Annuity Resources section at the bottom of the page.

Each form begins with the Annuity Forms Information Cover Sheet, which contains our contact information, including mailing addresses and fax number. This cover sheet explains the process of form submission. If you have questions regarding forms, or have an old form version and would like the latest version, you can contact our Annuity Contact Center at 1-800-862-7155 Monday through Thursday, 8:00 a.m. to 7:00 p.m., or Friday, 9:15 a.m. to 6:00 p.m., Eastern Time.
